Before you come to see us for your initial consultation, please take a few minutes to fill out the required health history form. It only takes a few minutes and will not only save time at your initial appointment but will also help us make sure you get the maximum value from your initial consultation. This appointment usually takes about 50 minutes. We would appreciate it if you would arrive a few minutes early to complete any additional paperwork. If you wish for us to help you with insurance, be sure to bring your dental insurance information—it is becoming more common for plans to offer benefits for orthodontic treatment.
